Lymphedema Bandaging and Manual Therapy Same Day

Recently, I have been receiving emails from my followers as well as seeing an increased discussion on social media platforms regarding the billing and payment of CPT code 97140 (manual therapy) on the same day the same provider also bills CPT code 29581 and/or 29584 (application of a multi-layer compression system). So what is the issue with payment of CPT code 97140 when performed on the same date of service that the provider also bills CPT code 29581 and/or 29584?

  1. I’ve always billed my Lymphedema bandaging under 97140 – are people being reimbursed 29581 for bandaging and are they able to bill it twice – once for each leg or is it a timed code?

  2. CMS article A52959 states that Medicare will only pay for 3 visits of High Compression bandaging for lymphedema, however Complete Decongestive Therapy is still a covered service. This seems contradictory since compression bandaging is one of the 4 components of CDT (manual lymphatic drainage, therapeutic exercise, skin care, and compression bandaging).
    Questions:
    1. Does this article also refer to short stretch compression bandages such as Comprilan®. Rosidal K®, etc or only to multiplayer compression systems such as Coban®, Profore®, Dyna-Flex®, Surepress®, Setopress®
    2. If it does not refer to short stretch compression bandages, should we be billing 97140, 29581/84 or something else?
    3. If it does not refer to short stretch compression bandages, are we able to bill for more than 3 visits?
