Benefit Policy Manual

The Medicare Benefit Policy Manual replaces current Medicare general coverage instructions that are not National Coverage Determinations. In general, these instructions have been found in Chapter II of the Carriers Manual, Intermediary Manual, and the various provider manuals, and in Program Memoranda. Material from the Coverage Issues Manual is in the National Coverage Determinations Manual (Pub. 100-3).
Chapter 1, Section 110-110.5 contains the rules and regulations for inpatient rehabilitation facilities.
Chapter 7 contains the rules and regulations relevant to home health services provided under a home health plan of care.
Chapter 8 contains the rules and regulations relevant to skilled nursing facilities providing care under Part A Medicare benefits.
Chapter 12 contains the rules and regulations relevant to services provided by comprehensive outpatient rehabilitation facilities.
Chapter 15, Section 220-230.6 contains the rules and regulations relevant to outpatient physical, occupational, and speech-language pathology services provided in all outpatient settings.
