Are you able to clarify what is meant by
“Reviewed with and approved by the treating physician in accordance with applicable state laws and regulations”. Does this mean signed and returned by the referring practitioner or would a prescription suffice?
I would recommend you contact the insurance carrier and ask them what they mean by that statement.
