Several weeks ago, I wrote an article titled “Required Elements of a Plan of Care” for Medicare Part B therapy services. This week, I want to discuss the plan of care required elements for Aetna, Cigna, UnitedHealthcare and several state Blue Cross/Blue Shield insurance carriers.

  1. Are you able to clarify what is meant by
    “Reviewed with and approved by the treating physician in accordance with applicable state laws and regulations”. Does this mean signed and returned by the referring practitioner or would a prescription suffice?

    1. I would recommend you contact the insurance carrier and ask them what they mean by that statement.
