2025 Therapy Code List Annual Update
The Centers for Medicare and Medicaid Services (CMS) has published the 2025 Therapy Code List. This list provides outpatient therapy providers with the CPT codes most commonly billed by physical therapists, occupational therapists, and speech-language pathologists. This list will tell providers which CPT codes are considered “always therapy” services and which CPT codes are considered “sometimes therapy” services.

I am receiving WPS/Medicare denials stating charges are adjusted based on multiple surgery rules or concurrent anesthesia rules. I am coding the same as I had been in 2024 and we were not receiving this ERA denial. Has something changed? I use the modifier GP to indicate Physical Therapy. Any ideas of what I might be doing incorrectly?
